Which is the correct form to be used while addressing a couple:
Mr & Mrs. 'woman's name'
Mrs & Mr. 'man's name'
Mrs & Mr. 'woman's name'
Mr & Mrs. 'man's name'

Some prefer Ms. to Mrs. (note that I say "some")

Correct form is
Mrs. & Mr. Man's Name
This is internationally accepted norm

Trial = Mrs. & Mr. John Smith
For some reason I have never heard that being used. The popular custom in North America is Mr. and Mrs. last name (as e pointed out) or Mr. and Mrs. Man's full name. Hmmm what would one do if the husband and wife have different last names?
I guess the best thing is to ask the couple how they would like to be addressed.
